What to do:
- Fill in the online form with basic details about the issue. (Please don’t include any personal or patient-identifiable information.)
- You’ll receive an email with a short questionnaire.
- Complete the questionnaire so our team can review your case.
- We’ll contact you with next steps, including whether you need to return the product.

Please note:
- To prevent damage during transport, please pack the product securely.
- If possible, please send the product with all accessories.
- If a loan product has been requested (Applies to UK only):
Along with the loan product, you will receive all the documents you need to return your product. Be sure to include the completed service form with your return or identify the product with the ACS processing number). - To make it easier for you to return your loan product, you will receive a return label with it.
